Insurance & Payment Information

In California, you can begin physical therapy without a physician referral. This is called direct access.
If treatment continues beyond 45 calendar days or 12 visits, whichever comes first, a physician, surgeon or podiatrist must review and approve the physical therapy plan of care.
Insurance requirements may vary, so please check whether your plan requires a referral for reimbursement.

Dr. Ariel Stern is an out-of-network provider offering personalized, one-on-one pelvic health physical therapy in the comfort of your home.
If you have PPO insurance, I can provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement. Coverage varies by plan, so I recommend contacting your insurance provider ahead of time to confirm your benefits.
You may also want to ask your insurance company what documentation they require and whether pelvic physical therapy is covered under your out-of-network benefits.

Payment is due at the time of service. Accepted payment methods include cash, check, HSA/FSA cards and credit cards. A 3.0% processing fee is added to credit card transactions.
Personalized Pelvic Health Care at Home
Your first visit is a private, unrushed appointment focused on understanding your pelvic health concerns, your symptoms and how they affect your daily life. We will talk through your history, goals, and any concerns related to bladder or bowel function, pelvic pain, postpartum recovery, intimacy or pelvic pressure.
The evaluation is guided by your comfort level, and everything is explained before it happens. By the end of the visit, you will have a clearer understanding of what may be contributing to your symptoms and a personalized plan for care.

Ongoing visits build on your initial evaluation and are focused on helping you make steady, meaningful progress. Each session is tailored to your symptoms, goals and how your body is responding to treatment.
Care may include pelvic floor training, guided movement, strengthening, mobility work, manual therapy, symptom-management strategies and updates to your home program. Visits are usually scheduled weekly or every other week, depending on your needs.
